A sample of 1.5 inches consisting of around 200 hair strands (comparable to a #2 pencil's diameter) nearest the scalp yields 100mg, which is optimal for initial and confirmatory testing. In the case of EtG testing, additional panels, or tests exceeding 10 panels, a specimen weighing 150mg is optimal. It's advisable to use a jeweler's scale for measuring the sample. If scalp hair cannot be obtained, an equivalent quantity of body hair is an option. The term "head hair" specifically pertains to scalp hair, while "body hair" includes all other hair types (e.g., facial, axillary).

Process Overview

The primary stages in laboratory handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the initial phase, incorporating the entry of a specimen into the lab's database. It involves ensuring the sample's seal and shipment compliance, assigning a random LAN (Laboratory Accessioning Number), and finalizing any extra data not covered by an electronic chain of custody system.

The Screening phase is a preliminary review for illicit substances. Although Screening is an economical means to exclude drug use for most samples, any positive screen result must be validated for court acceptability. Samples that indicate potential positivity in Screening must undergo additional confirmation.

Should a sample show possible positivity during Screening, extra hair is extracted from the initial specimen for the Extraction phase. Here, drugs are isolated from hair at much lower concentrations relative to other methods (e.g., urine or saliva), reinforcing why hair drug testing is notably challenging to execute.

For any positive screening, Confirmation is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples indicative of positivity are cleaned before confirmation as necessary. The complete lab protocol from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
