Hair collection is straightforward, somewhat challenging to tamper with, and convenient for shipping.

A 1.5-inch sample containing roughly 200 hairs (comparable to the diameter of a #2 pencil) near the scalp yields 100mg, which is ideal for analysis and confirmation. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of hair is advised. The specimen should be weighed using a jeweler’s scale. If scalp hair is not an option, a comparable amount of body hair can be collected. The term "head hair" exclusively refers to scalp hair. "Body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial processing stage where a sample is entered into a lab's system. It involves ensuring that the sample is sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supplied by an electronic chain of custody system.

Screening is a preliminary check for substance abuse. While Screening is a cost-efficient means of eliminating drug use in most samples, confirmed results are necessary for legal admissibility. Samples that test presumptively positive during Screening require further confirmation.

If a sample appears positive during Screening, additional hair is extracted from the original specimen for the Extraction stage. Drugs are extracted at much lower concentrations than in methods like urine or oral fluid, rendering hair drug screening one of the most intricate methodologies to execute.

Confirmation of positive results from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed as required before confirmation. The entire laboratory process from Accessioning through Confirmation is subjected to review by the College of American Pathologists (CAP) Hair designation, as well as accreditation to ISO/IEC 17025 standards.
