When collected near the scalp, hair provides an approximately 3-month detection period for alcohol and other drugs. Collecting hair is straightforward, somewhat resistant to tampering, and simple to transport.

A 1.5-inch sample with approximately 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp yields 100mg of hair, the perfect sample size for both screening and confirmation. For EtG, extra tests, and/or over 10-panel tests, a 150mg specimen is advised. We suggest using a jeweler’s scale for weighing the specimen. If scalp hair is not available, a similar quantity of body hair can be collected. When mentioning head hair, it strictly refers to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test result in the laboratory involves these four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial processing of a sample within the laboratory’s system. This step involves confirming the sample was correctly sealed and shipped, assigning an arbitrary LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not covered by an electronic chain of custody system.

Screening is the preliminary rapid assessment for drugs of abuse. While Screening offers a budget-friendly means to dismiss drug use in the majority of samples, a positive screen must be confirmed for court admissibility. Any samples that are tentatively positive during Screening require secondary confirmation.

If a sample results in a tentative positive during the Screening phase, additional hair is extracted from the initial specimen for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than seen in other procedures (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The completed laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
