Process Overview

Accessioning pertains to initially processing a sample into the lab system. This step confirms the sample was properly sealed and dispatched, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data not provided electronically.

Screening is a preliminary examination for drugs of abuse. Though Screening is cost-effective in excluding drug use for most samples, a positive initial finding must be verified for court acceptability. Any sample with a presumptive positive result in Screening requires a further confirmatory test.

Should a sample return a presumptive positive during Screening, additional hair is extracted from the initial sample and prepared for Extraction. This step involves extracting drugs from hair at significantly lower concentrations than with other methods (e.g., urine or oral fluids), making hair drug screening particularly challenging.

Confirming any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation, if necessary. The complete laboratory process from Accessioning to Confirmation complies with CAP (College of American Pathologists) Hair standards and ISO/IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
