However, for recent drug use within the past 5 days, urine testing remains more precise.

What it is, How it works

Hair testing has emerged as an effective means for identifying drug and alcohol usage. Hair retains a prolonged history of alcohol and drug intake by capturing biomarkers in the fibers of the developing hair strand. When obtained near the scalp, hair can deliver a detection period of up to around 3 months for substances. Hair is straightforward to collect, relatively hard to alter, and simple to ship.

A 1.5-inch portion of approximately 200 hair strands (similar in size to a #2 pencil) adjacent to the scalp will produce 100mg of hair, the perfect sample for both screening and confirmation. For EtG, add-ons, and/or examinations exceeding 10 panels, a 150mg specimen is advised. We suggest weighing the sample on a jeweler’s scale. If scalp hair isn't available, a comparable quantity of body hair may be used. When addressing head hair, we mean scalp hair specifically. Body hair includes all other varieties (facial, axillary, and more).

Process Overview

The core steps in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a lab’s system. This step confirms the sample was sealed and sent correctly, allocates a unique LAN (Laboratory Accessioning Number), and concludes any supplementary data entry absent from an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of misuse. While Screening is a budget-friendly approach to exclude drug usage on numerous samples, a favorable screen must be validated to be admissible legally. Any samples that are presumptively positive in Screening need further confirmation.

If a sample is found presumptively positive in the Screening phase, additional hair is taken from the original specimen and set for Extraction. In this phase, drugs are extracted from hair at much reduced concentrations than with other techniques (e.g., urine or oral fluid), showcasing hair drug screening as the most complex technique to administer.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as needed. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the certification to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
