For EtG, additional tests, or those exceeding 10 panels, 150mg of sample is advised. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair isn’t available, an equivalent amount of body hair can be gathered. Head hair specifically refers to scalp hair, whereas body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The process for laboratory handling of a drug test result consists of Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial incorporation of a sample into a lab’s system. This process ensures the sample was properly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and finishes any additional data processing not available via an electronic chain of custody system.

Screening entails a preliminary quick assessment for substances of abuse. Despite Screening being an affordable method to dismiss drug use on most samples, a positive screen requires confirmation to be legally valid. Any presumptively positive samples in Screening need additional confirmation.

If a sample is presumptively positive during Screening, additional hair is extracted from the initial sample and made ready for Extraction. In this phase, drugs are removed from hair at much lower levels than through other methods (e.g., urine or oral fluid), which makes hair drug analysis the most complex method to execute.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation if needed. The entire laboratory procedure from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
