A 1.5-inch piece made up of roughly 200 hairs (comparable to a number 2 pencil's width) cut closest to the scalp yields 100mg, the preferred amount for screening and confirmation. For EtG, add-ons, or tests above 10 panels, 150mg is advised. We advise using a jeweler’s scale for measuring. If scalp hair isn't an option, a similar amount of body hair can be used. "Head hair" strictly refers to scalp hair, while "body hair" encompasses other types like facial or axillary hair.

Process Overview

The core steps in lab processing a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts the integration of a sample into the lab's system. This means checking that it was sealed and sent correctly, assigning it a LAN (Laboratory Accessioning Number), and filling in any missing data not included by an electronic chain of custody.

Screening is a preliminary check for substance abuse. It's an efficient method to dismiss drug use in most samples, but any positive result must be verified to be valid in legal contexts. Any samples that indicate positive during Screening must be confirmed with further tests.

Should a sample test positive during Screening, additional hair is taken from the initial sample for Extraction. At this stage, substances are removed from hair at lower concentrations than in other tests (like urine or oral fluid), making hair drug screening a challenging methodology.

GC/MS, GC/MS/MS, or LC/MS/MS is used for the Confirmation of any positive screening outcomes. All samples considered presumptively positive are washed before Confirmation if necessary. The lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair norms and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
