A hair sample of 1.5 inches containing about 200 strands (around the size of a #2 pencil) closest to the scalp gives 100mg of hair, the optimal sample for screening and confirmation. For EtG, additional screenings, and/or tests beyond 10 panels, 150mg of the sample is advisable. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is inaccessible, an equivalent quantity of body hair can be used. When mentioning head hair, it specifically denotes scalp hair. Body hair includes any other hair types (facial, axillary, etc.).

Process Overview

The principal steps in the laboratory processing of a drug test outcome involve Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into the lab’s system. This process verifies that the sample was appropriately sealed and shipped, generates a random LAN (Laboratory Accessioning Number), and completes any extra data entry not available via an electronic chain of custody system.

Screening provides an initial swift check for drugs of abuse. While Screening is a cost-efficient approach to eliminate drug use in most samples, a positive screen must be validated to be legally admissible. Any samples that test positive in the Screening phase require a secondary confirmation.

If a sample is positive in the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. At this stage, drugs are retrieved from hair at a substantially lower concentration than with other methods (e.g., urine or oral fluid), which positions hair drug screening as the most complex methodology to execute.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed as necessary before confirmation. The complete lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.
