If scalp hair is not available, a comparable quantity of body hair may be used. When referencing head hair, it pertains exclusively to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into the laboratory's system. This process involves ensuring the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening involves an initial rapid analysis for substances of abuse. While Screening offers a cost-effective method to eliminate drug use in most samples, a positive screen requires confirmation to be legally valid. Any samples preliminarily positive in Screening need a secondary confirmation.

Should a sample show likely positive results during Screening, additional hair is taken from the initial specimen for Extraction. In this phase, drugs are extracted from hair at a notably lower concentration than other methodologies (e.g., urine or oral fluid), highlighting why hair drug screening remains the most challenging methodology to execute.

The confirmation of any positive Screening outcome is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The whole laboratory process, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
