Hair testing has emerged as a significant method for identifying drug and alcohol consumption. Hair captures a comprehensive record of alcohol and drug use by embedding biomarkers in the fibers of the growing hair strand. When obtained near the scalp, hair can offer a detection span of up to three months for alcohol and other drugs. Hair is easy to collect, hard to adulterate, and straightforward to transport.
A 1.5-inch hair sample of approximately 200 strands (roughly the width of a #2 pencil) closest to the scalp provides 100mg of hair, considered the optimal sample for analysis and validation. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of the specimen is advised. It is suggested to weigh the sample on a jeweler’s scale. If hair from the scalp is not accessible, an equivalent quantity of body hair can be gathered. By 'head hair', we specifically mean scalp hair. 'Body hair' refers to all other hair types (e.g., facial, axillary).
Process Overview
The four fundamental phases in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.
Accessioning involves initially processing a sample into the laboratory’s system. This step entails verifying that the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not supplied by an electronic chain of custody system.
Screening is an initial rapid test for abuse drugs. Even though Screening is a cost-efficient method to exclude drug use in most samples, a positive screen requires confirmation to be legally admissible. Samples that are preliminarily positive in Screening need subsequent confirmation.
If a sample shows presumptive positivity during the Screening stage, additional hair is extracted from the original specimen and prepared for Extraction. During this stage, drugs are extracted from hair at a much lower concentration than in methods like urine or oral fluid, which is why hair drug screening is considered the most challenging method to perform.
Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to confirmation if needed. The whole laboratory process, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.
