What it is, How it works

Hair captures a long-standing record of substances by embedding biomarkers within the strands as they develop. When sourced near the scalp, hair offers roughly a 3-month timeframe for detecting alcohol and other drugs. Hair is easily collected, hard to tamper with, and straightforward to transport.

A sample of 1.5 inches consisting of approximately 200 hairs (comparable to a #2 pencil's thickness) nearest to the scalp provides 100mg of hair, which is the optimal sample for examination and verification. For EtG, supplementary tests, and tests beyond 10 panels, a 150mg specimen is preferable. We suggest measuring the sample on a jeweler’s scale. If scalp hair isn't available, a comparable portion of body hair can be used. When we mention head hair, it pertains to scalp hair solely. Body hair encompasses all other hair types (such as facial or axillary hair).

Process Overview

The primary steps in the laboratory handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of recording a specimen into the laboratory’s system. This step ensures the sample was correctly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data input absent from an electronic chain of custody system.

Screening involves an initial examination for drugs of abuse. While Screening is an economical method to eliminate drug use in most samples, a positive outcome requires confirmation to be legitimate in legal settings. Any samples that test positive during Screening mandate a subsequent confirmation.

If Screening results are positive, additional hair is extracted from the initial sample for Extraction. This step involves extracting drugs from hair at a lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging process to execute.

Confirmation of a positive screening result is conducted using GC/MS, GC/MS/MS or LC/MS/MS techniques. Any presumptive positive samples undergo a washing process before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.

Forensic Applications and Interpretation: One of the vital objectives of hair testing is its application in forensic investigations and legal proceedings. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
