A 1.5-inch sample containing about 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for both screening and confirmation processes. For EtG, add-ons, and/or screenings beyond 10 panels, 150mg of the sample is advised. It is recommended to weigh the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be collected. Reference to head hair pertains solely to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily encompasses four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial incorporation of a sample into a lab's system. This process verifies that the sample was correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and addresses any additional data entry not covered by an electronic chain of custody system.

Screening offers a swift initial evaluation for abusive drugs. It serves as a cost-efficient method to rule out drug use in most samples; however, a positive screen must be substantiated to be accepted in court. Samples yielding presumptive positive results in Screening necessitate a secondary confirmation.

Presumptive positive samples from the Screening phase are subject to further hair extraction from the initial specimen for Extraction. In this step, drugs are isolated from hair at considerably lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening a particularly challenging technique.

Confirmation of positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positives undergo washing before confirmation as necessary. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
