Process Overview

The laboratory procedures for processing drug test outcomes consist of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary process of integrating a sample into a lab's database. This step ensures the sample is properly sealed and shipped, allocates a random LAN (Laboratory Accessioning Number), and finishes any extra data input not managed by an electronic chain of custody network.

Screening functions as a preliminary assessment for substances of abuse. Despite being a cost-efficient method to exclude drug usage in most samples, a positive initial result requires validation to stand in legal settings. Samples that test positive in Screening must undergo further verification.

If a sample tests positive during Screening, additional hair is taken from the original specimen and readied for Extraction, where substances are isolated from hair in much lower concentrations than other methods (e.g., urine or oral fluid), hence why hair analysis is notably challenging.

Verification of positive screening outcomes is achieved using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All samples deemed presumptively positive undergo washing before confirmation as needed. The complete lab process from Accessioning to Confirmation is evaluated under CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
