What it is, How it works

When sampled from near the scalp, hair can offer a detection window of up to roughly 3 months for alcohol and drugs. Hair is straightforward to collect, tough to tamper with, and convenient to transport.

A 1.5-inch segment, composed of about 200 hair strands, approximately the thickness of a #2 pencil, nearest to the scalp provides 100mg of hair, the perfect sample for both screening and confirmation. For EtG, additional panels, or tests exceeding 10 panels, 150mg of the sample is advised. The specimen should be weighed on a jeweler’s scale. In the absence of scalp hair, an equivalent amount of body hair can be used. "Head hair" refers solely to scalp hair, while "body hair" includes all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test outcome includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of processing a sample within a laboratory's system. This step ensures the sample was appropriately sealed and transported, allocates a random LAN (Laboratory Accessioning Number), and completes any pending data entry absent from an electronic chain of custody system.

Screening comprises a preliminary fast assessment for drug abuse. Though it's a cost-efficient technique to dismiss drug use in many samples, any positive screen must undergo confirmation before it can be legally upheld. Samples showing a presumptive positive in Screening necessitate a follow-up confirmation.

In cases where a sample is presumptive positive during Screening, additional hair is extracted from the original sample for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), thereby making hair drug testing one of the most challenging methodologies.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. Each step of the laboratory process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
