What it is, How it works

Hair analysis is recognized as an effective method for identifying drug and alcohol consumption. Hair captures a comprehensive record of alcohol and drug use by embedding biomarkers in the strands of the growing hair. When collected near the scalp, hair can offer a detection window for alcohol and drugs of up to approximately 3 months. The collection of hair is straightforward, relatively resistant to tampering, and simple to transport.

A sample measuring 1.5 inches with approximately 200 strands (comparable to the size of a #2 pencil) near the scalp yields 100mg of hair, which is the preferred sample size for testing and confirmation. For EtG, add-ons, and tests exceeding 10 panels, 150mg is advised. We suggest using a jeweler's scale for weighing the sample. If scalp hair is not accessible, a similar amount of body hair can be collected. When mentioning head hair, it solely refers to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed within a laboratory's system. This step verifies the sample was securely sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry that an electronic chain of custody system may not provide.

Screening entails an initial rapid assessment for substances of abuse. While Screening is an economical way to eliminate drug usage in most samples, a positive screen must be verified for legal admissibility. Samples that are presumptively positive in Screening require a secondary confirmation.

If a sample is presumptively positive during the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a minuscule concentration compared to other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Any positive screening outcomes are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are washed before confirmation as needed. The entire laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
