The adoption of hair follicle drug tests has surged recently, gaining preference over traditional urine tests. Many employers, legal institutions, and substance abuse professionals mandate hair drug testing due to its advantages. This method is favored by employers with zero-tolerance drug rules, courts, and probation services. A key advantage of hair testing is its ability to detect drug use for up to 90 days, ensuring collection credibility and resisting tampering. However, urine tests remain superior for identifying recent drug use within the past 5 days.

What it is, How it works

Hair analysis is recognized as an effective method for identifying drug and alcohol consumption. Hair captures a comprehensive record of alcohol and drug use by embedding biomarkers in the strands of the growing hair. When collected near the scalp, hair can offer a detection window for alcohol and drugs of up to approximately 3 months. The collection of hair is straightforward, relatively resistant to tampering, and simple to transport.

A sample measuring 1.5 inches with approximately 200 strands (comparable to the size of a #2 pencil) near the scalp yields 100mg of hair, which is the preferred sample size for testing and confirmation. For EtG, add-ons, and tests exceeding 10 panels, 150mg is advised. We suggest using a jeweler's scale for weighing the sample. If scalp hair is not accessible, a similar amount of body hair can be collected. When mentioning head hair, it solely refers to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed within a laboratory's system. This step verifies the sample was securely sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry that an electronic chain of custody system may not provide.

Screening entails an initial rapid assessment for substances of abuse. While Screening is an economical way to eliminate drug usage in most samples, a positive screen must be verified for legal admissibility. Samples that are presumptively positive in Screening require a secondary confirmation.

If a sample is presumptively positive during the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a minuscule concentration compared to other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Any positive screening outcomes are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are washed before confirmation as needed. The entire laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection period: Hair drug tests can identify drug use for up to 90 days, in contrast to urine tests, which have a shorter detection period.
  • Hard to deceive: Cheating a hair drug test is very challenging, thus enhancing result accuracy.
  • Offers historical insight: It can reveal a drug use pattern over time, rather than indicating only recent use.

Limitations:

  • Cannot identify immediate use: Drugs become detectable in hair approximately 5-7 days after use.
  • Expense: Hair drug tests are typically more costly than other drug testing methods.
  • Result variability: Factors such as hair color and individual differences in hair growth can influence drug metabolite concentration in hair.

Note: Although frequently called "hair follicle tests," the analysis focuses on the hair strand rather than the follicle beneath the scalp.
