In cases where scalp hair is unavailable, an equivalent amount of body hair can be gathered. "Head hair" refers exclusively to scalp hair, whereas "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s framework. This phase includes ensuring that the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry absent from an electronic chain of custody system.

Screening constitutes a preliminary rapid evaluation for drug abuse. While Screening serves as a cost-efficient method to rule out drug use in most samples, a positive screen requires confirmation to be valid in court. Any samples that exhibit presumptive positivity in Screening necessitate additional confirmation.

Should a sample show presumptive positivity during Screening, additional hair is extracted from the original sample and prepared for Extraction. In this phase, drug compounds are retrieved from hair at far lower concentrations compared to other techniques (e.g., urine or oral fluid), accentuating the complexity of hair drug screening.

The Confirmation of any presumptive positive results is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positivity undergo washing before Confirmation as needed. The comprehensive laboratory process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
