What it is, How it works

Hair screening has emerged as a robust method for identifying drug and alcohol consumption. Hair captures a comprehensive history of alcohol and drugs by embedding biomarkers within the fibers of the maturing hair strand. When collected near the scalp, hair offers up to an approximate 3-month detection period for alcohol and various drugs. Hair is straightforward to gather, somewhat challenging to tamper with, and simple to transport.

A 1.5-inch section of roughly 200 hair strands (similar to the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the perfect sample for both screening and verification. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We propose weighing the specimen using a jeweler’s scale. If scalp hair cannot be obtained, a proportional amount of body hair might be acquired. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four major stages in the laboratory analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial intake of a sample into a lab’s system. This includes confirming that the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. While Screening is a cost-efficient approach to eliminate drug consumption on the majority of samples, a positive screen requires confirmation to be acceptable in court. Any samples that presumptively test positive in Screening require additional confirmation.

In instances where a sample tests presumptively positive during Screening, more hair is extracted from the initial sample and prepped for Extraction. During this step, drugs are isolated from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to execute.

Verification of any positive screening result is achieved via GC/MS, GC/MS/MS or LC/MS/MS. All samples testing presumptively positive are cleansed before confirmation as needed. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
