For EtG, additional tests, and/or tests over 10 panels, a specimen of 150mg is suggested. It is advisable to measure the specimen using a jeweler’s scale. If scalp hair is not accessible, a comparable quantity of body hair can be obtained. When discussing head hair, it refers only to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial integration of a sample into a lab’s system. This process includes checking that the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and performing any additional data entries not handled by an electronic chain of custody system.

Screening involves a preliminary scan for substances. While Screening is a cost-efficient method to eliminate drug use in the majority of samples, a positive result requires confirmation to be deemed admissible in legal settings. Any samples showing presumptive positives in Screening necessitate further confirmation.

If a sample yields a presumptive positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair at lower concentrations compared to other methods (such as urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation, as needed. The entire laboratory procedure from Accessioning to Confirmation is overseen per both the CAP (College of American Pathologists) Hair designation and the accreditation of ISO / IEC 17025 standards.
