Reference to head hair pertains solely to scalp hair, whereas body hair encompasses other types (such as facial or axillary hair).

Process Overview

The laboratory procedure for processing a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s system. This involves ensuring the sample’s proper sealing and shipment, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any data entry not handled by an electronic chain of custody system.

Screening provides an initial rapid check for substance abuse. Although Screening is economically efficient for excluding drug use in the majority of samples, a positive result here necessitates confirmation for legal admissibility. Any sample that is preliminarily positive in Screening mandates a secondary validation.

If a sample shows preliminary positivity in the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. In this phase, drug substances are isolated from hair at significantly lower concentrations than other methods (for example, urine or oral fluid), which explains why hair screening is the most challenging form of drug testing.

Verification of any positive screening finding is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Any samples presumed positive are cleansed before confirmation if needed. The complete laboratory process from Accessioning to Confirmation undergoes evaluation under the guidance of both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
