What it is, How it works

Hair examinations have become recognized as an effective method for detecting the presence of drugs and alcohol. When gathered from near the scalp, hair samples can offer a detection span of approximately three months for consuming alcohol and other substances. Collection of hair is straightforward, resisting tampering, and can be easily transported.

A sample measuring 1.5 inches and about 200 hair strands (comparable to the thickness of a #2 pencil) collected nearest the scalp provides 100mg of hair, which is optimal for initial screening and verification. For EtG, additional tests, and/or panels over 10, a quantity of 150mg is advisable. We suggest utilizing a jeweler's scale to measure the sample. If scalp hair is not available, a similar volume of body hair can be used. When we mention head hair, we specifically indicate scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary phases in laboratory processing for drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory's system. This step verifies the sample's proper sealing and shipping, assigns a unique LAN (Laboratory Accessioning Number), and completes any missing data entry absent from an electronic custody framework.

Screening is an initial rapid assessment for substance misuse. Though Screening is an economical method to eliminate the possibility of drug use for most samples, a positive screen must undergo confirmation for legal admissibility. Samples yielding a presumptive positive during Screening require further confirmation.

For samples showing presumptive positivity during Screening, additional hair is extracted from the original collection for Extraction. During this phase, drugs are removed from hair at significantly lower concentrations than with alternate techniques (such as urine or oral fluid), making hair drug testing the most challenging procedure to execute.

Confirmation of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to confirmation, all samples that are presumptively positive undergo washing. The entire lab procedure, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair category and is accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
