For EtG, additional tests, and/or panels exceeding ten, 150mg of the sample is advised. It's suggested to weigh the sample using a jeweler's scale. If scalp hair is not obtainable, a similar quantity of body hair can be collected. When mentioning head hair, the reference is solely to scalp hair. Body hair includes all other hair types (facial, underarm, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of processing a sample into a laboratory's system. This encompasses ensuring the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody system.

Screening is an initial rapid assessment for drug abuse. While it is a cost-effective method to dismiss drug use in most samples, a positive screen necessitates confirmation to be valid in legal settings. Samples that return presumptive positive results in Screening require a secondary confirmation.

If a sample shows presumptive positivity in Screening, additional hair is extracted from the initial sample and prepared for Extraction. In this stage, drugs are isolated from hair at considerably lower concentrations than those in other techniques (e.g., urine or oral fluid), highlighting why hair drug screening is among the most challenging methodologies.

Confirmation of any positive screening outcomes is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation adheres to the oversight of both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
