What it is, How it works

A sample measuring 1.5 inches with around 200 hair strands (comparable to the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal amount for both screening and confirmation. For EtG, add-ons, and/or evaluations exceeding 10 panels, 150mg of the sample is suggested. It's recommended to weigh the sample using a jeweler’s scale. Should scalp hair be unavailable, a corresponding quantity of body hair can be gathered. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample within a laboratory’s framework. This covers confirming the sample’s seal and appropriate shipment, assigning a unique LAN (Laboratory Accessioning Number), and completing extra data entry tasks absent from an electronic chain of custody platform.

Screening constitutes a preliminary rapid assessment for drug misuse. While screening is an economical method to dismiss drug consumption in the majority of samples, a positive screen must be authenticated for legal admissibility. Any samples that are tentatively positive during Screening indeed necessitate a secondary verification.

If a sample yields a presumptive positive during the Screening phase, additional hair is extracted from the initial specimen and prepped for Extraction. In this phase, drugs are extracted from hair at a considerably lower concentration than other methods (e.g., urine or oral fluid), making hair drug assessment the most challenging method to execute.

Validation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples suspected to be positive undergo washing prior to confirmation as required. The holistic laboratory procedure, from Accessioning through to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair certification and the standard of ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
