What it is, How it works

Hair analysis has become recognized as an effective means for identifying drug and alcohol consumption. Hair encapsulates a chronological record of alcohol and substances through the capture of biomarkers within its growing strands. When sampled near the scalp, hair offers a detection period of up to three months for alcohol and drugs. Collecting hair is straightforward, somewhat resistant to tampering, and easy for transportation.

An ideal sample consists of a 1.5-inch section with around 200 hair strands (comparable to the width of a #2 pencil) near the scalp, yielding about 100mg of hair, suitable for both initial testing and confirmation. For EtG, additional panels, or testing over 10 panels, a 150mg sample is advisable. It's suggested to weigh the sample using a jeweler’s scale. In the absence of scalp hair, a corresponding amount of body hair can be used. References to head hair specifically pertain to scalp hair. Body hair is defined as any other types of hair (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of a sample into a lab’s workflow. This step ensures that the sample is properly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and handles any additional input not covered by an electronic chain of custody.

Screening offers an initial rapid assessment for substance abuse. While cost-effective for ruling out drug use in most samples, a positive screen must be verified for legal admission. All presumptive positives from Screening necessitate further confirmation.

For samples that test presumptive positive during Screening, additional hair is extracted from the initial sample and prepped for Extraction. In this phase, substances are isolated from hair at much lower concentrations compared to other analysis methods (like urine or oral fluid), highlighting hair drug testing as the most challenging method to execute.

Verification of any positive screening outcome occurs through GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive samples undergo washing before confirmation, as necessary. The complete laboratory sequence from Accessioning to Confirmation is evaluated according to the CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
