Process Overview

The laboratory procedure for drug test results encompasses four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Within Accessioning, there is an initial process where a sample is integrated into the laboratory's system. This phase ensures the sample’s proper sealing and shipment, assigns an arbitrary LAN (Laboratory Accessioning Number), and performs additional data entry when not handled by an electronic chain of custody system.

In the Screening phase, an expedient initial assessment for drugs of abuse is conducted. While Screening is a cost-efficient strategy to eliminate drug use on most samples, a positive screen mandates confirmation to hold up in court. All samples that test presumptively positive in Screening necessitate follow-up confirmation.

Presumptively positive samples from the Screening must undergo further processing in the Extraction stage, where additional hair is extracted from the initial sample. This stage involves extracting drugs from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which renders hair drug screening a highly challenging process.

For Confirmation, positive screening results are validated through GC/MS, GC/MS/MS, or LC/MS/MS. Before Confirmation, all presumptive positive specimens are washed as needed. The entire laboratory workflow, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
