What it is, How it works

Hair testing is considered a valuable method for identifying drug and alcohol usage. Hair captures a prolonged record of alcohol and drug consumption by embedding biomarkers in the strands of growing hair. When hair is gathered near the scalp, it can offer up to a 3-month window for detecting alcohol and drugs. Hair is straightforward to collect, relatively hard to tamper with, and easy to transport.

A 1.5-inch sample consisting of roughly 200 hair strands (about the circumference of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the ideal quantity for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen weighing 150mg is suggested. We advise weighing the sample on a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair may be utilized. When we mention head hair, we mean scalp hair exclusively. Body hair includes all other types like facial, axillary, etc.

Process Overview

The drug test involves four essential phases in the laboratory process: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of integrating a sample into a laboratory’s system. This step ensures the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid test for drugs of abuse. Screening is a cost-effective solution to dismiss drug usage in most samples; however, a positive screen requires confirmation for court presentation. Samples that test presumptive positive in Screening need to undergo further confirmation.

Should a sample test presumptive positive during Screening, additional hair is taken from the original specimen and prepared for Extraction. During this phase, drugs are separated from the hair at a much lower concentration than other methods (such as urine or oral fluid), making hair drug screening a challenging methodology to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed as needed prior to confirmation. The entire laboratory process from Accessioning to Confirmation is overseen under CAP (College of American Pathologists) Hair guidelines and meets the accreditation standards of ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
