What it is, How it works

Hair serves as a historical record of alcohol and drug intake by encapsulating biomarkers within the fibers of the growing strands. When gathered near the scalp, it can offer a detection window for approximately up to three months for substances. Collecting hair is straightforward, challenging to tamper with, and convenient to transport.

A 1.5-inch sample comprising roughly 200 hair strands (equivalent to the size of a #2 pencil) taken closest to the scalp provides 100mg of hair, which is ideal for screening and validation. For EtG, extended panels, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest using a jeweler’s scale for specimen weighing. If scalp hair is not available, an equivalent amount of body hair may be utilized. In discussions of head hair, we specifically mean scalp hair. Body hair encompasses all other hair forms (such as facial or axillary hair).

Process Overview

The drug test result processing in a laboratory involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the lab's system. This step includes ensuring the sample was correctly sealed and delivered, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry not managed by an electronic chain of custody system.

Screening involves an initial rapid assessment for drug abuse. Although Screening is a cost-efficient method to exclude drug use in most samples, any positive result requires confirmation to be legally admissible. Samples that yield a presumptive positive result in Screening need secondary confirmation.

If Screening indicates a presumptive positive result, additional hair is extracted from the original specimen for the Extraction phase. Drugs are extracted from the hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging to execute.

Confirmation of any positive screening results is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with a presumptive positive result are washed prior to confirmation as needed. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and under ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
