What it is, How it works

Hair testing is now regarded as a robust method for identifying drug and alcohol consumption. Hair serves as a long-term indicator of alcohol and other drug use by capturing biomarkers within the strands of growing hair. When taken near the scalp, hair provides a detection period of up to three months for alcohol and other drugs. It is straightforward to gather, relatively hard to tamper with, and easy to transport.

A sample of 1.5 inches of around 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is ideal for screening and confirmation. For EtG, supplementary tests, and/or tests exceeding 10 panels, a 150mg sample is suggested. We advise measuring the sample on a jeweler's scale. If scalp hair isn’t available, a similar amount of body hair could be used. When referencing head hair, we mean scalp hair only. Body hair refers to all other types (facial, axillary, etc.).

Process Overview

The lab analysis of a drug test result encompasses four fundamental steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of a sample into a lab's system. This step includes ensuring the sample has been appropriately sealed and transported, attributing a random LAN (Laboratory Accessioning Number), and completing any extra data not provided by an electronic chain of custody system.

Screening includes an initial rapid assessment for drug use. While it is a cost-efficient method to exclude drug use in most samples, a positive screen must be verified to be valid in court. Samples that are presumptively positive in Screening require a secondary confirmation.

If a sample is presumptively positive in the Screening stage, more hair is extracted from the original specimen for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (for instance, urine or oral fluids), which makes hair drug testing one of the most challenging methodologies to conduct.

Confirmation for any positive screen result is done using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that are presumptively positive are cleansed before confirmation as needed. The whole laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
