A sample measuring 1.5 inches, comprising about 200 hair strands (approximating the thickness of a #2 pencil) from the scalp can yield 100mg, which is the ideal quantity for both screening and confirmation. For tests including EtG, add-ons, and those exceeding 10 panels, 150mg of the hair specimen is advised. It's suggested to use a jeweler’s scale for precise weighing of the sample. If scalp hair is not available, equivalent body hair can be used. When we refer to head hair, we mean scalp hair exclusively. Body hair encompasses other types (facial, axillary, etc.).

Process Overview

The four primary stages in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial incorporation of a sample into a lab’s system. This step involves confirming the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and filling in any additional data entry absent from an electronic chain of custody system.

Screening provides a preliminary check for drug abuse. While Screening is an economical approach to exclude drug use for most samples, a positive outcome requires confirmation to be valid in court. All samples that yield a presumptive positive result necessitate secondary confirmation.

If the Screening phase indicates a presumptive positive, additional hair is extracted from the original specimen for Extraction. During this phase, drugs are extracted from hair at lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug testing the most challenging methodology to execute.

Confirmation of any positive result from the screening is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples receive a wash before confirmation as needed. The complete laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
