What it is, How it works

Hair testing is recognized for its efficacy in identifying drug and alcohol consumption. Hair offers a substantial record of alcohol and drug use by embedding biomarkers in the hair strand's fibers as it grows. When taken from near the scalp, it can unveil up to approximately a 3-month timeframe for alcohol and drug detection. The collection of hair is straightforward, relatively tamper-resistant, and convenient for transport.

A 1.5-inch segment comprising roughly 200 hair strands (approximately the girth of a #2 pencil) near the scalp yields 100mg of hair, ideal for screening and verification. For EtG, additional tests, and/or beyond 10 panels, 150mg of the sample is advisable. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair isn’t available, an equal quantity of body hair can be used. When we mention head hair, it specifically means scalp hair. Body hair covers all other hair forms (facial, axillary, etc.).

Process Overview

The four fundamental stages in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a lab's system. This involves ensuring the sample was correctly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data input absent from an electronic chain of custody record.

Screening encompasses an initial rapid assessment for drugs of abuse. Although Screening efficiently rules out drug usage for most samples, a positive screen necessitates validation for legal purposes. Samples deemed presumptively positive during Screening require a secondary confirmation.

If a Screening step indicates a presumptive positive, additional hair is extracted from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at substantially lower concentrations than other techniques (e.g., urine or oral fluid), highlighting why hair drug screening is very complex to perform.

Confirmation of a positive screening outcome is completed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are washed as necessary. The full lab procedure from Accessioning through Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 certification standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
