A 1.5-inch portion comprising roughly 200 strands (about the size of a #2 pencil) from the scalp area yields 100mg, the optimal quantity for preliminary analysis and validation. For EtG, additional tests, and panels beyond 10, a 150mg sample is suggested. Using a jeweler's scale is advised to measure the specimen accurately. If scalp hair isn't available, an equivalent volume of body hair may be obtained. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (e.g., facial, axillary).

Process Summary

The laboratory analysis of a drug test comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially recorded into a lab's system. This step ensures the sample is securely sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and finalizes any data entry not covered by an electronic chain of custody system.

Screening serves as a preliminary evaluation for abusive substances. Though cost-efficient to exclude drug use in most samples, any positive outcome from Screening necessitates further verification for legal proceedings. Samples indicating possible positivity in Screening require subsequent confirmation.

If positivity appears during Screening, additional hair is extracted for preparation in the Extraction stage. Here, substances are retrieved from hair at considerably lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Verification of positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All suspected positive samples are cleaned before confirmation as required. The entire lab workflow from Accessioning through Confirmation undergoes scrutiny per the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
