What it is, How it works

Hair testing has become recognized as an effective method for identifying drug and alcohol usage. Hair offers an extended historical record of alcohol and drug intake by encapsulating biomarkers in the strands as they grow. When gathered near the scalp, hair can provide an approximate 3-month window of detection for alcohol and drugs. Hair is straightforward to collect, not easily tampered with, and simple to transport.

A 1.5-inch section of around 200 strands of hair (comparable in size to a #2 pencil) nearest to the scalp yields 100mg of hair, the preferred sample size for both screening and confirmation. For EtG, additional panels, and/or over 10-panel tests, 150mg of the sample is advised. It's recommended to weigh the sample using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent amount of body hair can be used. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The process in the lab for developing a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the first step of inputting a sample into the lab system. This process includes confirming that the sample was appropriately sealed and transported, allocating a random LAN (Laboratory Accessioning Number), and completing any remaining data entry absent from an electronic chain of custody system.

Screening involves an initial assessment for drugs of abuse. Though Screening is a cost-efficient technique to dismiss drug use in most samples, a positive screen must undergo confirmation to be considered court-worthy. Samples that are initially positive in Screening need a further confirmation.

If a sample initially tests positive during the Screening phase, additional hair is extracted from the initial sample for preparation in the Extraction process. Here, drugs are extracted from hair at significantly lower concentrations than other approaches (like urine or oral fluid), making hair drug screening a challenging method to administer.

Positive screening results are validated through GC/MS, GC/MS/MS, or LC/MS/MS. All initial positive samples undergo washing before confirmation if required. The complete laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.
