A hair segment of 1.5 inches from approximately 200 strands (roughly the size of a #2 pencil) near the scalp results in 100mg, the optimal amount for analysis. For EtG, additional evaluations, or tests exceeding 10 panels, a 150mg sample is advised. Utilize a jeweler’s scale for specimen measurement. If scalp hair is unavailable, a similar quantity of body hair may be used. By head hair, we mean only scalp hair, whereas body hair encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test samples involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning integrates a sample into the laboratory’s workflow, ensuring proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary manual data entry.

Screening serves as an initial assessment for drugs, an economical method to exclude drug use in most specimens. However, a positive screen necessitates confirmation for court acceptance. Presumptive positive samples from Screening require further verification.

For samples indicating presumptive positivity during Screening, additional hair is extracted for Extraction. During this step, substances are separated from hair at lower concentrations compared to other methods (e.g., urine or saliva), which makes hair testing more complex.

Positive screening outcomes are validated using GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positives undergo washing before confirmation as appropriate. The complete laboratory process, from Accessioning through Confirmation, adheres to the CAP (College of American Pathologists) Hair standards and meets ISO/IEC 17025 accreditation requirements.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
