If head hair is unavailable, an equivalent amount of body hair may be used. When we mention head hair, we refer specifically to scalp hair. Body hair covers all other types (facial, axillary, etc.).

Process Overview

The primary stages in the laboratory handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a specimen into a lab's system. This process includes ensuring that the specimen was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing extra data entry not covered by an electronic chain of custody system.

Screening involves an initial rapid examination for drugs of abuse. Though Screening is a cost-efficient method to eliminate drug usage in the majority of specimens, a positive screen must be confirmed for legal admissibility. Any specimens presumptively positive in Screening must undergo a secondary confirmation.

If a specimen is presumptively positive during the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are drawn out from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening outcome is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleaned before Confirmation as needed. The full laboratory process from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
