A sample of 1.5 inches consisting of around 200 hair strands (comparable to the thickness of a #2 pencil) taken from the scalp yields 100mg of hair, which is ideal for both screening and confirming. For EtG, additional tests, or those exceeding 10 panels, a 150mg sample is advised. Using a jeweler’s scale to weigh the sample is recommended. If scalp hair is inaccessible, an equivalent amount of body hair can substitute. Reference to head hair indicates only scalp hair; body hair encompasses all other types (such as facial or axillary hair).

Process Overview

The lab processing of a drug test involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab's system. This step ensures the sample is properly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional information entry not covered by an electronic chain of custody system.

Screening serves as a preliminary check for substance abuse. Although efficient for excluding drug use in most samples, a confirmed positive result is necessary for legal proceedings. Presumptive positive samples during Screening require further validation.

Should a sample show a presumptive positive during Screening, additional hair is obtained from the initial sample for the Extraction phase. During Extraction, the drug concentration extracted from hair is notably lower compared to other methods (e.g., urine or oral fluid), making hair drug screening the most complex technique to execute.

Any positive result from screening is verified via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed as needed before confirmation. The complete laboratory sequence, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
