For tests involving EtG, additional elements, or panels exceeding 10, 150mg of the sample is advised. We suggest using a jeweler’s scale for weighing the sample. Should scalp hair be unavailable, a proportional amount of body hair can be collected. When discussing head hair, we exclusively refer to scalp hair. Body hair encompasses all other types (such as facial and axillary hair).

Process Overview

The primary steps of examining a drug test in the lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial incorporation of a sample into a laboratory’s system, involving verifying appropriate sealing and shipment of the sample, allotting a random Laboratory Accessioning Number (LAN), and completing any pending data inputs not addressed by an electronic chain of custody protocol.

Screening offers an initial rapid assessment for potential drug abuse. While Screening provides a cost-efficient method to eliminate drug usage in most samples, a positive result necessitates confirmation to be valid in legal matters. Presumptively positive samples in Screening require follow-up confirmation.

If Screening reveals a presumptive positive result, additional hair is extracted from the initial sample and readied for Extraction. At this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most intricate to execute.

Confirmation of any positive screen outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All samples with presumptive positives undergo washing before confirmation, if necessary. The entire progression from Accessioning to Confirmation is evaluated under the College of American Pathologists (CAP) Hair designation and accredited to ISO / IEC 17025 guidelines.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
