For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is advisable. Utilizing a jeweler’s scale to measure the specimen is recommended. In scenarios where scalp hair isn't accessible, an equivalent amount of body hair can be gathered. When mentioning head hair, we denote solely scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary phases in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab’s system. This entails ensuring proper sealing and shipment of the sample, allotting a random LAN (Laboratory Accessioning Number), and fulfilling any additional data entry absent from an electronic chain of custody system.

Screening consists of an initial rapid assessment for drugs of abuse presence. Although Screening is a cost-efficient method to eliminate drug use on most samples, a positive result must be confirmed to hold up in court. Samples that are presumptively positive in Screening require additional confirmation.

If a sample appears positive in the Screening stage, more hair is extracted from the initial sample for Extraction. In this phase, drugs are removed from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which explains why hair drug screening is extremely challenging to execute.

Confirmation of any positive screening outcome is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
