What it is, How it works

When taken from just above the scalp, hair can offer an up to approximately 3-month detection timeframe for alcohol and other drugs. Hair is easy to collect, hard to tamper with, and simple to transport.

A 1.5-inch sample of roughly 200 strands of hair (similar to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal sample for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advisable. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair is unobtainable, a similar amount of body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four principal steps in the laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the preliminary processing of a sample into a laboratory’s system. This involves ensuring the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry not offered by an electronic chain of custody system.

Screening involves an initial rapid check for drugs of abuse. Although Screening is a cost-effective method to exclude drug usage in the majority of samples, a positive screen must be confirmed to be permissible in court. Any samples that are presumptively positive in Screening do necessitate a secondary confirmation.

If a sample is presumptively positive during the Screening step, additional hair is extracted from the original specimen and readied for Extraction. In this phase, drugs are drawn out from hair at a considerably lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as needed. The entire laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
