The drug test laboratory process encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the preliminary integration of a sample into the laboratory’s framework. This step ensures the sample’s correct sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any additional manual data entry absent from an electronic custody chain system.

Screening entails an initial rapid assessment for drug presence. While cost-effective for excluding drug usage in most samples, a positive result in Screening necessitates further validation for courtroom acceptability. Samples indicating presumptive positives in Screening require confirmatory analysis.

For specimens showing presumptive positivity during Screening, additional hair from the initial collection is prepared for the Extraction stage. This phase involves isolating drugs from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screenings particularly challenging to execute.

All positive outcomes in screening undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive specimens are pre-washed as required before Confirmation. The comprehensive lab process, from Accessioning to Confirmation, aligns with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
