What it is, How it works

Hair testing has emerged as a potent method for uncovering alcohol and drug consumption. Hair captures a long-term record of substance use by embedding biomarkers within the hair's fibers as it grows. When obtained near the scalp, hair can offer up to nearly a 3-month detection period for both alcohol and drugs. Collecting hair is easy, fairly hard to tamper with, and convenient to transport.

A 1.5-inch segment of approximately 200 hair strands (roughly the diameter of a #2 pencil) cut closest to the scalp yields 100mg of hair, the optimal amount for thorough screening and verification. For EtG, additional requirements, and/or examinations above 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale for weighing the sample. If no scalp hair is available, a similar quantity of body hair can be gathered. When discussing head hair, we specifically mean scalp hair. Body hair includes all other hairy regions (face, armpits, etc.).

Process Overview

The primary stages in the laboratory processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial handling of a sample within the lab’s framework. This step confirms that the sample is sealed and shipped correctly, allocates a random LAN (Laboratory Accessioning Number), and fulfills any further data input absent in an electronic chain of custody system.

Screening refers to an initial swift analysis for substances. This method is cost-efficient for dismissing drug presence in most samples; however, a positive result requires further confirmation for legal credibility. All samples preliminarily positive during Screening must undergo an additional confirmation step.

If a sample is preliminarily positive in the Screening phase, more hair is separated from the original sample and readied for Extraction. This stage involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which explains why hair drug analysis is among the most complex methodologies to execute.

Confirmation of any positive result from screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive need to be washed before confirmation as required. The comprehensive lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair certification and according to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
