What it is, How it works

When obtained near the scalp, hair can deliver up to a roughly three-month detection window for alcohol and other substances. Hair is straightforward to obtain, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch sample comprising around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal amount for both screening and confirmation. For EtG, additional tests, and/or tests beyond 10 panels, a specimen of 150mg is suggested. We advise checking the specimen weight with a jeweler’s scale. If scalp hair is not present, a comparable quantity of body hair can be used. The term head hair specifically refers to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The key steps in processing a drug test result in a lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial entry of a sample into a laboratory’s system. This step verifies that the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not included by an electronic chain of custody system.

Screening is an initial rapid test for drugs of abuse. Although Screening is a budget-friendly way to exclude drug use in most samples, a positive screen must be confirmed to be valid legally. Samples that are presumptively positive during Screening require further confirmation.

If a sample tests positive during the Screening phase, more hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at much lower concentrations compared to other techniques (e.g., urine or oral fluid), thus making hair drug screening the most challenging technique to execute.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accrediting standards.
