For EtG, additional analyses, or tests exceeding 10 panels, 150mg is advisable. We suggest using a jeweler’s scale for weighing. If scalp hair isn't available, a matching amount of body hair is acceptable. "Head hair" refers solely to scalp hair, while "body hair" encompasses all other types (such as facial or axillary hair).

Process Overview

Accessioning covers the initial integration of a sample into a lab’s framework. This involves ensuring the sample was sealed and dispatched correctly, assigning it a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry omitted by an electronic chain of custody system.

For presumptive positives during Screening, additional hair is harvested from the primary sample and processed for Extraction. In this phase, substances are extracted from hair at remarkably lower concentrations compared to other methods (e.g., urine or saliva), which is why hair drug analysis is the most complex methodology to conduct.

Confirmation for any positive result in screening employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptively positive samples undergo a washing prior to confirmation as necessary. The complete laboratory process, from Accessioning through to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair accreditation and the ISO / IEC 17025 standards certification.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
