When cut close to the scalp, hair can offer up to a roughly three-month detection span for alcohol and other substances. Hair is straightforward to collect, somewhat challenging to tamper with, and convenient to transport.

A 1.5-inch segment of approximately 200 strands (similar in size to a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the preferred sample size for evaluation and verification. For EtG, additional tests, and/or screenings over 10 panels, 150mg of the sample is advised. We suggest weighing the sample on a jeweler’s scale. If scalp hair isn’t available, a comparable amount of body hair can be gathered. When mentioning head hair, we mean scalp hair exclusively. Body hair stands for all other hair forms (facial, axillary, etc.).

Process Overview

The key steps included in a laboratory’s processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample into the lab’s framework. This involves validating that the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any extra data input not supplied by an electronic chain of custody setup.

Screening involves a preliminary fast assessment for drugs of misuse. While Screening is a budget-friendly approach to eliminate drug use in most samples, a positive result in this step must be corroborated for court admissibility. Any samples that show a presumptive positive in Screening require further confirmation.

In cases where a sample shows a presumptive positive in Screening, additional hair is drawn from the initial sample and prepped for Extraction. In this step, drugs are isolated from hair at a significantly lower concentration than in other approaches (e.g., urine or oral fluid), making hair drug analysis the most intricate technique to execute.

Verification of any positive screen result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
