What it is, How it works

Hair testing has emerged as an effective approach for identifying drug and alcohol consumption. Hair can record a long-term history of alcohol and drugs by embedding biomarkers within the strands growing from the head. When taken from near the scalp, hair offers approximately a 3-month window for detecting alcohol and other substances. Hair collection is straightforward, challenging to tamper with, and easy to transport.

A 1.5-inch section consisting of around 200 hair strands (comparable in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, an optimal quantity for testing and verification. For EtG, additional tests, or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest measuring the sample on a jeweler’s scale. If scalp hair cannot be obtained, an equivalent quantity of body hair may be used. In referring to head hair, we mean scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory process for evaluating a drug test result consists of four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary introduction of a sample into a lab’s system. This incorporates confirming the proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening provides an initial, swift evaluation for substance abuse. Although Screening is a budget-friendly method to exclude drug usage in most samples, a positive screen must be validated before it is legally permissible. Samples that test positive initially in Screening require subsequent confirmation.

When a sample shows a presumptive positive during the Screening stage, additional hair is extracted from the initial supply and prepared for Extraction. Within this phase, drugs are extracted from hair at a significantly lower concentration than other approaches (such as urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Any positive screening result undergoes Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as presumptively positive are cleansed before confirmation as necessary. The comprehensive lab process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
