What it is, How it works

Hair testing has gained recognition as an effective method for identifying drug and alcohol consumption. Hair retains a long-term record of alcohol and drug use by embedding biomarkers in the fibers of the growing hair strands. Collected near the scalp, it provides a detection window of up to three months for substances. Hair collection is straightforward, difficult to tamper with, and easy to transport.

A 1.5-inch sample consisting of around 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp yields approximately 100 mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, or tests beyond 10 panels, a 150 mg specimen is advised. We suggest using a jeweler’s scale for weighing. If scalp hair is not available, a similar amount of body hair may be used. "Head hair" refers specifically to scalp hair, while "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The key steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is entered into a laboratory's system. This phase includes checking that the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not available through a digital chain of custody system.

Screening is a preliminary rapid check for drug use. While cost-effective, a positive screen requires confirmation for legal admissibility. Samples testing positive in Screening necessitate secondary confirmation.

If Screening indicates a presumptive positive, additional hair is extracted from the original sample for the Extraction phase. In this phase, drugs are extracted at lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to conduct.

Confirmation of any positive screen is done via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are washed as necessary before confirmation. The entire process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
