A 1.5-inch section of roughly 200 hair strands (akin to the thickness of a #2 pencil) collected close to the scalp yields 100mg of hair, which is the optimal sample size for analysis and confirmation. For extended tests, such as EtG add-ons or tests exceeding 10 panels, a 150mg sample is preferred. It's advisable to measure the sample using a jeweler's scale. Should scalp hair be lacking, an equivalent amount of body hair can be used. 'Head hair' pertains solely to scalp hair, while 'body hair' includes all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of drug test results encompasses four core steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially logged into a lab system, which involves confirming the sample's proper sealing and shipment, assigning a LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic custody system.

Screening provides a preliminary examination for drugs. Although this is a cost-effective strategy to exclude drug use in most cases, a positive Screening result must be validated for legal proceedings. Samples with positive Screening results undergo a secondary confirmation.

Presumptive positives identified at the Screening stage result in additional hair being extracted from the original sample for the Extraction process. In this step, drugs are isolated from hair at lower concentrations than those used in other methods (e.g., urine or oral fluid tests), making hair drug screening the most challenging method to execute.

GC/MS, GC/MS/MS, or LC/MS/MS is used to confirm any positive screening findings. Any samples testing positive are washed before confirmation as necessary. The complete lab processing from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation.
