Collected near the scalp, hair can indicate up to roughly a 3-month detection period for alcohol and other drugs. Hair is straightforward to gather, somewhat challenging to tamper with, and straightforward to transport.

A 1.5-inch sample comprising about 200 hairs (about the width of a #2 pencil) closest to the scalp provides 100mg of hair, the optimal sample for examination and verification. For EtG, add-ons, and/or assessments surpassing 10 panels, 150mg of the specimen is advised. We suggest using a jeweler’s scale for weighing the specimen. If scalp hair is absent, the acquisition of an equivalent quota of body hair is possible. When "head hair" is mentioned, it refers solely to scalp hair. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The four primary steps involved in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into a lab’s system. This ensures the sample was sealed and dispatched correctly, allocates a random LAN (Laboratory Accessioning Number), and finishes any additional data processing not furnished by an electronic chain of custody system.

Screening involves an initial quick examination for drugs of misuse. While Screening is an economical method to dismiss drug use in most samples, a positive screen must be corroborated for court admissibility. Any samples that are provisionally positive in Screening necessitate a secondary verification.

If a sample appears presumptive positive during the Screening phase, more hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at much lower concentrations than alternative methods (ex. urine or oral fluid), which makes hair drug testing the most intricate methodology to execute.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All provisionally positive samples are cleansed prior to confirmation as necessary. The whole lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the endorsement to ISO / IEC 17025 standards.
