A 1.5-inch portion of roughly 200 hair strands (comparable in size to a #2 pencil) nearest to the scalp provides 100mg, which is the optimal amount for both primary screening and confirmation. For EtG, additional options, or tests with more than 10 panels, 150mg is the suggested sample mass. It's advisable to measure the sample using a jeweler’s scale. If scalp hair isn't available, a similar quantity of body hair can be collected instead. When discussing head hair, it specifically refers to scalp hair, while body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a laboratory’s system. This step includes confirming that the sample was appropriately sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any ancillary data entry not provided by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drug presence. While Screening is a cost-efficient method to rule out drug usage in most samples, any positive result needs confirmation for legal admissibility. Samples suspected of being positive during Screening necessitate secondary confirmation.

If a sample is deemed potentially positive in the Screening phase, additional hair is extracted from the original sample for Extraction preparation. During this stage, drugs are removed from the hair at significantly lower concentrations than seen in other testing methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most complex to conduct.

Confirmation of any presumptive positive from Screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples showing presumptive positivity are cleansed before confirmation as necessary. The complete laboratory process, from Accessioning to Confirmation, is conducted under the CAP (College of American Pathologists) Hair classification and adheres to ISO / IEC 17025 accreditation.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
