What it is, How it works

A 1.5-inch segment consisting of around 200 hair strands (approximately the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample size for screening and verification. For EtG, add-ons, and/or tests surpassing 10 panels, 150mg of the specimen is advised. We suggest measuring the specimen using a jeweler’s scale. If scalp hair is not accessible, a comparable quantity of body hair may be gathered. When discussing head hair, we specifically mean scalp hair. Body hair refers to all other hair types including facial and axillary hair.

Process Overview

The four primary phases in the lab's processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory’s system. This entails ensuring the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening encompasses an initial rapid assessment for drugs of abuse. Although Screening is an economical technique to exclude drug use in most samples, a positive screen must be validated to be admissible in legal proceedings. Samples that are presumptively positive in Screening require secondary confirmation.

If a sample is presumptively positive during the Screening stage, additional hair is extracted from the primary specimen and set for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a cleansing process before confirmation when necessary. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
