What it is, How it works

Hair testing is regarded as an effective method for identifying drug and alcohol consumption. Hair serves as a time-long record of alcohol and drugs by capturing biomarkers within the strands as they grow. When taken near the scalp, hair offers up to a roughly 3-month timeframe for detecting alcohol and drugs. Hair gathering is straightforward, hard to tamper with, and straightforward to transport.

A 1.5-inch hair sample, comprised of approximately 200 strands (about equivalent to a #2 pencil) nearest to the scalp, provides 100mg of hair, which is the perfect quantity for testing and verification. For EtG, additional tests, and/or panels exceeding 10, a specimen amount of 150mg is advised. It is recommended to measure the sample on a jeweler’s scale. In the absence of scalp hair, a comparable quantity of body hair can be gathered. 'Head hair' pertains solely to scalp hair. 'Body hair' encompasses all other hair forms (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test result mainly includes Accessioning, Screening, Extraction, and Confirmation stages.

Accessioning involves the initial input of a sample into a lab’s system. This ensures the sample was properly sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and completes any lacking data entry from an electronic chain of custody system.

Screening provides an initial rapid assessment for drug abuse. Although Screening is a cost-effective approach to exclude drug use in most samples, a court-admissible result requires confirmation of any positive screen. Samples that are tentatively positive in Screening necessitate further confirmation.

If a sample shows a tentative positive at the Screening phase, additional hair is extracted from the initial sample for Extraction preparation. In this step, drugs are removed from hair at lower concentrations than other techniques (e.g., urine or saliva), which makes hair drug testing the most complex method.

Any positive Screening findings are confirmed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before necessary confirmation. The lab process from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 standards accreditation.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
