What it is, How it works

A sample taken from near the scalp can encompass a detection period of up to roughly three months for substances like alcohol and drugs. Collecting hair is uncomplicated, challenging to tamper with, and straightforward to transport.

A 1.5-inch section of roughly 200 hairs (comparable to the width of a #2 pencil) from the scalp is needed for a complete sample, offering 100mg ideal for testing. For EtG, extended panels, or when exceeding 10 panels, a 150mg specimen is advisable. Utilizing a jeweler’s scale for weighing the specimen is advised. If scalp hair isn't accessible, a comparable volume of body hair can be used. When referencing head hair, only scalp hair is intended. Body hair encompasses all other growths (facial, axillary, etc.).

Process Overview

The laboratory processing for a drug test result encompasses four steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample first enters the laboratory system. This phase ensures the sample’s seal and shipment are intact, assigns a LAN (Laboratory Accessioning Number), and completes any missing data entry not covered by an electronic custody chain.

Screening acts as a preliminary assessment for drug presence. While cost-effective for most samples, a conclusive court-admissible result requires confirmation of any positive screen. Samples flagged as presumptively positive in Screening need secondary confirmation.

If Screening results are presumptively positive, additional hair is extracted from the specimen for the Extraction phase. Here, drugs are isolated from hair at significantly lower concentrations than other methods, such as urine or saliva testing, making hair drug testing the most complex technique to execute.

Confirmation of positive Screening results is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing, if necessary, before Confirmation. Every stage from Accessioning to Confirmation is reviewed according to the CAP (College of American Pathologists) Hair designation and compliant with the ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
